In consideration for participation in the Village of Barnwell Obstacle Course, the undersigned, on his or her own behalf, and on behalf of the participant(s) identified below, hereby acknowledges and agrees as follows:

1. I understand that participation on the aforementioned course carries inherent risk of injury, including, without limitation, minor injuries such as cuts, bruises and scrapes, as well as potentially severe injuries such as broken limbs, paralysis, or death. I knowingly and freely assume all such risk, both known and unknown, including those that may arise out of (i) the negligence or carelessness of other participants, or (ii) dangerous or defective equipment, on behalf of myself and those participants identified below.

2. With full understanding of the risks, and on behalf of myself and those participants named below, and our respective heirs, successors, and assigns, I hereby release and hold harmless The Village of Barnwell, and waive all claims for damage to person or property in connection with myself and those participants named below, for activities associated with any of the inflatables provided. This release and waiver hereby includes all officers, operators, employees, directors, agents, representatives, managers and affiliates of the Village of Barnwell, and their respective heirs, successors and assigns.

3. I certify on behalf of myself and those participants named below that all participants are physically fit and may participate in activities, and have not been advised otherwise by a qualified medical professional.

4. I agree that I, and the participants identified below, shall comply with all guidance and safety rules, signs and verbal instructions from The Village of Barnwell employees, and understand that failure of myself or the participants identified below to do so may result in expulsion from the course.
